What is a Release Authorization?

Clients in the state of New York wanting donor sperm to shipped directly to their home (home insemination) are required to have a doctor complete and sign a Release Authorization. This form is the doctor's approval for the specimen to be used for artificial insemination at the client’s place of residence. Clients for all other states do not require a Release Authorization.
