CONSENT TO EXTRACTION OF SPERM SAMPLE AND ASSESSMENT OF SUITABILITY AS A SPERM DONOR

With this consent form you consent to Cryos International ApS, company registration no. 35 51 42 52, Vesterbro Torv 3, 5., 8000 Aarhus C (Cryos) extracting your sperm sample for further processing and Cryos’ assessment of your suitability of becoming a sperm donor with Cryos based on personal data collected about you for that same purpose.

Your sperm sample contains genetic data, but we will only analyse the sample for the sperm cells’ ability to move. Your genetic data will not be retrieved or analysed. The result of the analysis includes health information about you.

If the result of the sperm analysis is satisfying, we will collect sensitive personal data about your race, ethnic origin, additional information to assess your health, including information about any illnesses and deformities in your family, as well as information about your sexual orientation.  

All this information is collected for the purpose of assessing your suitability as a sperm donor, cf. the more detailed description in Cryos’ Privacy Policy for Sperm Donor Candidates and Sperm Donors.

You can withdraw your consent at any time by sending an email to [email protected].

Any withdrawal of your consent will not affect Cryos’ processing of information that we are still obliged to keep in order to comply with legislation, or which we process as part of the performance of an agreement entered into between Cryos and you.
