The Extended Donor Profile Questionnaire is required to be completed by each Cryos donor. We ask that you put effort into completing the document with as much information as you can possibly add. Where appropriate, please answer the questions with a minimum of 4-5 sentences. Please feel free to write as much as you would like. We want your personality to shine through in this document. The Extended Donor Profile is a document that clients will review who are interested in purchasing your specimens.
Read the questionnaire thoroughly before starting. Most of the questions are mandatory and you will not be able to save your answers while completing the questionnaire.
- Begin only when you have the time and when you have all the information you need to complete the questionnaire.
- If you have all the information ready, it takes between 1 and 1½ hours to complete the form.
- Avoid including information that identifies you. For instance, if you write “Graduated Cornell University in 2008 with a major in Political Science”, this statement narrows the population for your donations to those females who graduated in 2008 from Cornell University with a major in Political Science. The appropriate way to respond to this question is “Graduated from a top university in the United States with a major in Political Science.
- It is important that you add a personal touch to the profile. Please elaborate where possible. Please answer with a minimum of 4-5 sentences where appropriate.
Example: Answer to a Question:
What childhood event left the biggest impression on you? "When I was ten years old, my grandparents took me and my sister camping for a whole week. We picked wild berries, played hide-and-seek in the forest and went swimming in the lake. Nights were spent around the bonfire and while enjoying the berry pies my grandmother had made, my grandfather told stories about the old days and about all the important things in life. This was a great bonding experience and it created a lasting memory of spending time with my grandparents."
Naturally, we will make sure that your identifying information (name, ID number, email address, etc.) is removed from the completed profile.
