CONSENT TO COLLECTION OF INFORMATION FOR THE ASSESSMENT OF SUITABILITY AS AN EGG DONOR

With this consent form you consent to Cryos International ApS, Company Registration Number 35 51 42 52, Vesterbro Torv 3, 5., 8000 Aarhus C, Denmark (Cryos) collecting sensitive personal data about you for the purpose of assessing if you are suitable of becoming an egg donor with Cryos. 

When applying to become an egg donor, Cryos will collect your name, date of birth, post code and country of residence and your contact information and donation site  as provided by you at the online application form.  

To determine your initial eligibility, Cryos will collect sensitive personal data about your race, ethnic origin, additional information to assess your health, including information about any illnesses and deformities in your family, as well as information about your sexual orientation, cf. the more detailed description in Cryos’ Privacy Policy for Egg Donor Candidates and Egg Donors. 

You can withdraw your consent at any time by sending an email to [email protected]. 

Any withdrawal of your consent will not affect Cryos’ processing of information that we are still obliged to keep in order to comply with legislation, or which we process as part of the performance of an agreement entered into between Cryos and you.
