Before you visit one of our departments, you must fill out our online sperm donor application.
Please note that you must be between 18 and 44 years old and live or work within one hour of one of our locations while being a donor at Cryos. We have sperm bank locations in Orlando, Raleigh, and Dallas.

2. Semen analysis
Once you have submitted your application, you must visit us to provide a preliminary sperm sample. We will get in touch with you to schedule this appointment.
We will analyze the sample to see if your sperm quality is good enough for you to become a sperm donor, and we will contact you with the results. If needed, we may ask you to provide 1-2 additional sperm samples.
To ensure good sperm quality, you should not have ejaculated in the last 2–3 days prior to making a sperm sample.

If your semen analysis shows your sperm quality is good enough, you will need to complete an online questionnaire about your health and a thorough background check. Our staff will then review your answers and assess whether you meet our sperm donor requirements. If you do, you will be asked to participate in a short phone interview where we will schedule you for the next step.

4. Clinical examination and first donation
Next, you will come back to one of our locations for a clinical examination where one of our healthcare professionals will collect urine and blood samples.
At this point, you will be considered preliminarily approved as a Cryos donor. Therefore, you will be able to provide your first sperm donation and receive compensation.
Please remember to bring a valid photo ID and your Social Security card.

Next, you will come back to one of our locations for a clinical examination where one of our healthcare professionals will collect urine and blood samples.
At this point, you will be considered preliminarily approved as a Cryos donor. Therefore, you will be able to provide your first sperm donation and receive compensation.

5. Final approval
If your urine and blood tests are satisfactory, you will receive final approval as a sperm donor. This means you can officially continue donating at Cryos while helping families in need.
